Risk Incident Reporting

What is Risk Incident?

Risk Incident is any unplanned specific event (or set of circumstances) which has occurred and:

 

  • has had, or could have had (“near miss”), a negative impact on NUS on various fronts (Financial, 
Human, Reputational, Legal, Business etc.)
  • could pose a threat to the University’s mission and/or negatively affect NUS’ chance of meeting its intended goals.
What to report?

Risk Incidents NOT reported on other existing NUS platforms/routes.

What are the examples?

  • Academic Misconduct (Students) – Cheating during assessments discovered (single or widespread)
  • Substance abuse by staff/students discovered
  • Athletic misconduct such as cheating discovered
  • PR incident – Controversial statement by NUS community (staff/student) resulting in negative media coverage, public outrage etc
  • Contractual dispute with NUS vendor or partners
  • Corporate Payment Scams/Fraud (E.g., Vendors fraudulently paid without receipt of goods/services or payment made to scammers impersonating a genuine vendor)
  • Vendors fraudulently paid without receipt of goods/services
  • Loss of key personnel (Faculty/Administration)
  • Key facilities/critical infrastructure failure (disrupting campus operations/posing safety issues to NUS community)
  • IP theft by external parties / Allegations of IP infringement by NUS staff/students
  • Lawsuits (potential / actual)
  • Food poisoning incident after dining at NUS dining facility
  • Vandalism on campus
  • Non-compliance with NUS Policy Documents discovered
